Hyaluronic acid injections
Osteo and Physio are pleased to offer Ostenil, a synthetic form of hyaluronic acid which offers a targeted approach to alleviate discomfort and improve mobility. This synthetic hyaluronic acid injection works directly within the joint space, aiming to restore the natural balance of synovial fluid.

Ostenil replenishes this lost hyaluronic acid, helping to reduce friction, decrease pain, and enhance joint function.

Corticosteroid Injections

Cortisone injections offer a powerful and targeted approach to alleviate discomfort and improve your quality of life. These injections, containing a corticosteroid (sometimes combined with a local anaesthetic), work to reduce inflammation directly at the source of your pain.

Cortisone is not a pain reliever itself, but a potent anti-inflammatory. It works by inhibiting the production of collagen, effectively suppressing the inflammatory response. This reduces swelling, redness, and irritation, calming the nerves and indirectly reducing pain. The local anaesthetic, when used, provides immediate but temporary numbness.
Cortisone injections are often used to manage pain associated with:

  • Arthritis (osteoarthritis, rheumatoid arthritis)
  • Bursitis
  • Tendonitis
  • Carpal tunnel syndrome
  • Other inflammatory joint and soft tissue conditions
  • Relief typically begins within a few days, with the full effects becoming apparent within a few weeks.
  • Numbness from the local anesthetic will subside within a few hours.
  • The duration of pain relief varies, ranging from several weeks to months, and in some cases, may resolve the issue entirely.
While cortisone injections are generally safe and effective, there are a number of side effects and contraindications which need to be taken into consideration. Your therapist will discuss this with you in detail before any injections are provided.

Joint Aspiration

Joint aspiration or ‘Arthrocentesis’, is a minimally invasive procedure that can provide both relief and valuable diagnostic information. This simple technique involves drawing fluid from your joint using a needle, effectively reducing swelling and pain.

The term “arthrocentesis” combines “arthro-” (joint) and “-centesis” (puncture with a needle). It’s a procedure where fluid is aspirated, or withdrawn, from a joint space like your knee, hip, elbow, or knuckle. This excess fluid often accompanies inflammation, contributing to discomfort and limited mobility.
Arthrocentesis is typically performed in your healthcare provider’s office or as an outpatient procedure. Here’s what to expect:
  • Preparation: The skin around the joint is sterilized, and a topical anesthetic is applied to numb the area.
  • Guidance: If necessary, imaging technology like ultrasound or fluoroscopy may be used to precisely locate the joint capsule.
  • Aspiration: A needle attached to a syringe is inserted into the joint, and fluid is carefully withdrawn. Multiple syringe changes may be needed.
  • Optional Injection: If required, medication, such as cortisone, platelet-rich plasma, or viscosupplementation, can be injected into the joint using the same needle and pathway.
  • Fluid Analysis: The aspirated fluid can be sent to a laboratory for analysis, providing valuable diagnostic insights.
  • Arthrocentesis offers both therapeutic and diagnostic benefits:
    • Pain Relief: Removing excess fluid reduces swelling and alleviates joint pain.
    • Preparation for Treatment: It creates space for therapeutic injections, such as cortisone, PRP, BMAC, or viscosupplementation.
    • Diagnostic Insights: Analyzing the aspirated fluid can help identify:
      • Infections (bacteria, high white blood cell count)
      • Injuries (joint bleeding)
      • Crystal deposits (gout, pseudogout)
The entire procedure typically takes around 10 minutes, with slight variations depending on imaging use or additional injections. Complications are rare but may include:
  • Infection
  • Bleeding from blood vessel injury
  • Allergic reactions to anesthesia

Ultrasound machines use a transducer (small hand-held device) to send high-frequency sound waves to the affected area. Echoing sounds are transmitted back to the transducer which then sends them to a computer for processing. Based on the information, an image of the target area is visualised in real time. This is extremely helpful when pain or injury is triggered by movement, which cannot be captured in a static image.
 
Ultrasound-guided injections use dynamic musculoskeletal ultrasound imaging to follow the movement of the needle inside your body and guide it to the correct location. This improves the accuracy of the procedure and makes it safer to treat pain, inflammation and impaired mobility.
 
Ultrasound technology has the added advantages of being non-invasive, painless, quick, without radiation and can be used in the presence of metal implants which can be problematic in MRI imaging.
